Clinic for maxillofacial Oral Surgery and Implantology

The Clinic for Maxillofacial Oral Surgery and Implantology is a modern, highly specialized healthcare facility dealing with the diagnosis and treatment of the most severe diseases of the face and the neck.                                                                                                          
The history of this Clinic is connected with a doctor by the name of Atanasije Puljo who, during the First Balkan War (in 1912), managed the Jaw Fractures Department established within the Fifth Reserve Hospital in Belgrade.
In 1972, the Clinic for Maxillofacial Surgery was separated from the Dental Clinic and headed by General Prof. Antonije Škokljev, the first holder of Ph.D. degree in maxillofacial surgery in the Yugoslav Armed Forces of that time.

 The Clinic for Maxillofacial Oral Surgery and Implantology has three departments:

1.    Department of Maxillofacial Surgery
2.    Department of Oral Surgery
3.    Department of Implantology

 Department of Maxillofacial and Oral Surgery and Implantology

Department of Maxillofacial and Oral Surgery and Implantology deals with virtually all the aspects of surgery in the areas of traumatology (war wounds, repositions and ostesynthesis of the   facial  bones), oncology (removal of benign and malignant tumors of the soft and bone tissue in the head and neck region, parotidectomy, various types of neck dissection, resection of the maxilla and mandible), orthognathic surgery ( partial and total dentoalveolar and skeletal osteostomy of the facial  bones and ostegenetic distractions), reconstructive surgery (posttraumatic and postresctional reconstruction of soft and bone tissue of the face by regional and distant cutaneous, myocutaneous and osteomyocutaneous free and vascularized grafts, free rib and iliac osteotransplants),   as well as with infections of deep structures in the face and neck region and the treatment of severe vascular malformations of the face and neck.

Oral Surgery Department

In addition to standard oral surgical interventions, the Department performs today all known surgical procedures in the filed of Oral Surgery. So, in the last 15 years, too many already known methods have been applied and improved along with the introduction of new original ones such as:

-    transoral monocortical wire osteosynthesis in lower jaw fructures;
-    hollowing of vestibular and lingual sulcus in extremely atrophic jaws;
-    modification of the Caldwell surgery of  hollowing of lingual sulcus in  extremely atrophic  mandible
-    Enlargement of alveolar bone in extremely atrophic jaws  by  replacement of a tooth with an implant  and reimplantation of a tooth                                                                                                                 
-    extraction of  impacted teeth;
-    surgical – orthodontic treatment;
-    surgeries of cysts of different etiologies in the  orofacial region;
-    surgeries of the jaw sinus;
-    resection of teeth with one or more roots (in upper jaw by transnatral method);
-    implantation of bone and mucous membrane replacements in chronic periapical and cystic changes and periodontitis;
-    oral and surgical treatment of patients with medical risks and,
-    extraction of teeth in patients undergoing continuous anticoagulant therapy.

The number of examinations and interventions provided by the Department of Oral Surgery in recent years amounts to 15 000 per year, of which more than 1 500 surgical procedures are performed in the operating room. A special emphasis is put on the treatment of diseases of the mouth and teeth in handicapped children and adults, which is provided in coordination with physicians from the Department of Pediatric and Preventive Dentistry and physicians from the Dental Clinic’s Department of Diseases of the Teeth and Endodontics. Those surgical procedures are performed under general anesthesia and require great effort and responsibility of all involved physicians.

Apart from the treatment of military patients, the Oral Surgery Department serves   civilian patient population as well.  They are seen on Mondays and Wednesdays within  the Department’s hours of operation between 03:30 p.m. and 07:30 p.m.

Implantology Department

Oral Implantology is a branch of Dentistry dealing primarily with dental implants inserted to improve chewing function, esthetics and phonation in partly or completely edentulous patients.

Dental implant is an artificial device fabricated or constructed in the Department’s laboratory and is surgically implanted into a jawbone to replace a part of the tooth, a missing tooth or teeth and provide permanent support for a fixed bridge or denture.    

Even though, Oral Implantology is the branch of Dentistry, which has been the last to be recognized, it is as old as Dentistry. Attempts made to replace missing teeth with dental implants date back to the ancient times. Archeological excavations has also revealed very rare samples of several thousand year old implants

The use of implants in dental practice started 35 years ago. There were attempts of introducing implantology into Dentistry over that period of time.  Tavčar, Škokljev and Spajić from the Military Medical Academy were those who implanted subperiostal implants into the lower toothles jaw in patients with advanced atrophy of a jawbone. In 1978, Alksandar Škundrić, another dentist from the MMA, used transdental implants of his own design. His name has been associated with continuous use of implants in the practice of Dental Clinic since 1981. He is also a creator of the homemade B.C.T. implant system, which has successfully been applied at the MMA for more than two decades.

Since 1977, the Military Medical Academy has continuously been using implants in its dental practice. In 1988, a separate Department of Clinical Implantology and Surgical Prosthetics was established. It operated within the framework of the Clinic for Maxillofacial Surgery and was headed by Aleksandar Škundrić. In 1998, that Department grew into the MMA’s Institute of Oral Transplantation and Implantology, and today.  operates as the Department of Implantotlogy. It has two sections:

-    Section for research, development and the use of implants and,
-    Section for periimplantology with a laboratory for testing and construction of bone substituents.

The Section for research, development and the use of implants is concerned primarily with implantation and full prosthetic treatment of a patient. Surgical team performs all kinds of surgical procedures required for the treatment of edentulous patients as well as atrophies of the jaws. The use of dental implants made of titanium provides the support for fixed prosthesis (crowns and bridges) which, in terms of function, suitability and esthetics, can not be compared with removable appliances (dentures). Implant crowns are usually made of ceramics.  

In addition to implantation, all other dental, periodontal, conservative and surgical procedures are performed to provide a full dental treatment to a patient. Complete treatment of caries and endodontic therapy prior to insertion of implants is provided at the Section for Periimplantology.  The Section has a Laboratory set up in 1998 for production and testing of bone substituents such as:

-    deproteinized human bone;
-    resorbed demineralized  collagen membrane, and  
-    platelet rich plasma  

Those products are used for surgical treatment prior to implantation, in treatment of periimplantitis, periodontitis and in the course of the implantation procedure

Ovi proizvodi laboratorije su patentirani u Zavodu za intelektualnu svojinu Srbije, od kojih je humana deproteinizovana kost (HDK) dobila „Grand prix“ na Izložbi pronalazaštva i patenata Beograd 2002, prvu nagradu Saveza pronalazača SRJ „Zlatna plaketa Tesla-Pupin“ za pronalazače godine 2002. i prvu nagradu „Tesla Fest“ za pronalazače godine 2003.
 
Implants are also used in cases of posttraumatic and posttumorous defects of the face and jaws for construction of various types of opturator prostheses and epitheses. The life span of implants is the best indicator of the success of an implantation procedure. So far, several thousands of implants of various types have been implanted at MMA. They were successful in more than 95% of cases.  
Such a large amount of work cannot be done without a teamwork and partnership with other institutions.

In addition to the right selection of a patient based on indications and contraindications of implantation, education of a patient regarding dental hygiene and regular check-ups is an equally important prerequisite for successful implantation.
